The Equipment Operator Competency Evaluation card is a two (2) part tri-fold form. Part 1 contains a detailed list of the E.O.'s responsibilities and the competency process. On the flip side, part 2, contains the competency evalution testing questions and skills demonstration criteria process with room to document the results for both.
